Medical Boarding Agreement

Boarders can be dropped off anytime during our hours of operation at 7am-8pm

Special boarding rates may apply during the holiday period of October 15 - January 15 and we will also collect a 50% deposit to guarantee your boarding reservation during the busy and preferential holiday season.

Parasites:

To ensure the safety of all our guests, it is mandatory that our boarders are parasite free. We will examine your pet(s) at drop off and treat for parasites (i.e. fleas, ticks, etc) as needed. If any fleas and/or ticks are observed on your pet(s) while boarding, they will be treated appropriately at your cost.*

Feeding Instructions/Diet:

Please provide packaged meals in pre-portioned Ziploc bags labeled with your pet's name to keep the pet's diet consistent to prevent gastrointestinal problems i.e. vomiting and diarrhea caused by the stress of boarding. Provide some extra bags in case of emergencies if your pet’s stay should be extended.

Should your pet experience gastrointestinal (GI) upset, we will make every attempt to notify you. While the exam is included in your boarding fee, all medications and special diets (ex. intestinal canned food and antibiotics) to treat GI upset will be at the pet parents' expense.*

Personal Belongings:

We will provide comfortable bedding for your pet. We discourage leaving personal belongings (i.e. toys, blankets, bedding, etc.) due to sanitation and orderliness requirements. We are not responsible for any items that are lost or soiled.*

Contagious Diseases:

At PetVet, we do all we can to prevent contagious diseases from coming into our facility and from spreading by ensuring proper immunizations, but PetVet does not make any guarantee that your pet will not be exposed to the disease.

Should your pet(s) contract a communicable disease (much like the common cold in child daycare) while boarding, PetVet will not be held liable. And similar to GI upset, all medications to treat these diseases will be at the pet parents' expense.

For the health of our general daycare population, we ask that you please notify us of any concerns prior to your boarding stay.*

Abandonment:

Please notify us if there are any changes in your pet's boarding plans. According to State Law, a pet is considered abandoned if we are unable to contact you 14 days after your pet’s scheduled release date.*

Photo Release Form:

I hereby grant to PetVet Animal Hospital, the irrevocable and unrestricted right to use and publish photographs of my pet(s), or in which they may be included, for editorial, trade, advertising, and any other purpose and in any manner and medium; to alter the same without restrictions and to copyright the same. I hereby release PetVet Animal Hospital from all claims and liability relating to said photographs.*

CPR:

In the event that your pet should experience cardiac or respiratory arrest while being hospitalized today, do you give consent for resuscitative efforts to be initiated until you can be contacted further and notified of your pet's status? By consenting to this service, you are also acknowledging that certain fees will apply. If you are not able to be contacted immediately, resuscitation efforts will be continued to be performed at the doctor’s discretion. Please select your choice below.**
